Social Work Services

Our Social Work staff helps patients and their families deal with the emotional, social and, at times, financial stresses that accompany illness and hospitalization.

They offer the following services:

  • assistance with discharge planning for home care or transfer to a nursing home or continuing care facility
  • information for the patient and or caregiver on housing, transportation, food, clothing, translation, financial, insurance, and legal concerns
  • referrals to external agencies in the areas of child and elderly abuse, domestic violence, adoption, community agencies, and organ and tissue donation;
  • emotional support and counseling related to advance directives.  

The primary role of the social worker is to enhance human well­-being and help meet the basic human needs of all people, with particular attention to the needs and empowerment of people who are vulnerable, oppressed, and living in poverty.

All social workers adhere to a strict code of ethics and have a working knowledge of social work theories, therapies, and techniques at a level acquired through the completion of a master's degrees in social work.
